- A team-based model of care led by a physician who provides coordinated and continuous care throughout a patient's lifetime
- The PCMH practice is responsible for providing for all of a patient's health care needs or arranging care with other qualified health professionals
- PCMH includes the provision of preventive services, treatment of chronic and acute illness, and assistance with end-of-life issues
- A practice in which a team of doctors works together to provide high levels of care, communication, care coordination and integration, and care safety and quality
